New Clients

Please download and complete the New Client Intake forms from our Insurance page in advance of your appointment. Remember to bring all completed forms with you on the day of the appointment, along with your insurance card and driver's license or ID. Thank you.

Payment

Insurance copays or appointment costs (for cash patients) are due at time of service. If you are using insurance, please contact your insurance provider in advance to verify your benefits. Elizabeth Bellmer is in-network with most of the major insurance carriers in Washington State. Please see the Insurance page for more information.

No-Show & Cancellation Policy

If you need to cancel or reschedule your appointment, please call (425) 406-7373 or email elizabethbellmercounseling@gmail.com at least 24 hours in advance to avoid a no-show/late cancellation fee. Insurance will not cover a missed appointment. No-show/late cancellation fees are the sole responsibility of the patient. Thank you for your consideration.
